Monthly Membership Program

When ordering any of our membership services, you will receive regularly scheduled automated payments after your initial order and you will be billed $19.95 per month to receive MD Plus Basic; $49.90 to receive MD Plus Ultimate; or $99.90 to receive MD Plus VIP, depending on the program you order for as long as you stay a member of our Membership program. The card you provided when you placed your initial order will automatically be billed for these charges. To modify your Membership, or to cancel future payments in the MD Plus Membership program, you must call 1-888-625-2629 at least 1 day prior to the date that your next monthly delivery ships. Our customer service center is open Mon-Fri: 9am - 5pm EST. 

Membership Agreement

By becoming a member in this program, you are authorizing MD Plus, Inc to bill your credit card, checking account, ACH draft or Pay-pal account for this program. This charge shall remain in force until you notify MD Plus, Inc. via telephone or in writing of its cancellation. By becoming a member in this program you are agreeing to the terms and conditions herein. This membership will automatically renew at the end of your membership term on a monthly basis, and your account will be automatically debited for your monthly fees. If you wish to change your payment method, please contact MD Plus, Inc, at the number indicated.

A fee paid for a membership becomes nonrefundable upon the expiration of the period for which it was applied.  The membership fees are paid by you for your access to the program for that month, regardless of your use of the service.   Once the month ends, you will not be able to reclaim, charge back or stop payment of the fee paid for that month.  Please contact customer service if you have any questions regarding the membership fees and requirements of this program.

TERMINATION: MD Plus, Inc. reserves the right to terminate or cancel the Membership of any Member or Member’s dependents or Members family or Members household or deny eligibility to any person for any reason, including a Member’s failure or refusal to pay for the program services, doctor payment, or for nonpayment of membership fees payable to MD Plus, Inc.

CANCELLATION POLICY: Please call MD Plus, Inc. to terminate or cancel your membership and automatically revoke your permission to continue any further payments. This membership can be cancelled for non-payment.

REFUND POLICY: If you cancel for any reason, any unused portion of your membership fee will be returned to you on a pro rata basis.  For example, if you cancel your membership after 6 months and 15 days, and you paid the monthly membership fee of 19.95 for the 7th month, then you would receive a refund of approximately $9.98.   Any additional refund or payment is based solely upon the discretion and good will of MD Plus, Inc.  PLEASE NOTE: Any consultation fee paid to a network doctor or facility is non-refundable under this agreement.  The Ambassador Fee is a non-commissionable and non-refundable fee of $199.

LIMITATIONS, EXCLUSIONS & EXCEPTIONS
Neither MD Plus, Inc, nor the administrator is licensed insurers, health maintenance organizations or underwriters of health care services. No portion of any provider’s fees will be reimbursed or otherwise paid by MD Plus, Inc or the Administrator.

ADDITIONAL BENEFITS: This program makes it possible for its members to receive membership benefits through participating pharmacies. These benefits cannot be used in conjunction with any other kind of rate reduction mechanisms. These benefits will vary between participating pharmacies.

These benefits are NOT INSURANCE, nor should it be in lieu of or to replace any existing health insurance. Members are responsible for payment in full at the time that the product and/or services are provided.

PHARMACIES: It is the member’s responsibility to confirm with membership services that a pharmacy is an active participant of the program prior to seeing that provider. The Administrator reserves the right to add, change, and/or eliminate any of the service providers without prior notice to the member. It is possible that a member may not receive a discount.

MEDICAL CONSULTATION:There is a $35.00 consultation fee every time you speak directly to a doctor. This Program, its affiliates, agents and sales representatives are not responsible for providing or failing to provide medical consultations or advise to a member or to a members dependents, family or household. Such decisions to accept medical consultation shall not be based on the advice, credentials or recommendations of this program, its affiliates, agents or sales representatives.

THIS PROGRAM DOES NOT PRACTICE MEDICINE NOR DOES IT INTERFERE, PARTICIPATE NOR FORM PART OF THE PROVIDER-DOCTOR/MEMBER-PATIENT RELATIONSHIP.

SERVICE AVAILABILITY: This program does not guarantee that the products, services or facilities are available in every geographic location. The terms “24 hour” and other time sensitive service commitments refer to the programs around the clock telephone access. While the Administrator will make all reasonable efforts to provide around the clock services, in some rare cases our systems may be temporarily unavailable. THIS PROGRAM IS NOT AN

EMERGENCY PROVIDER NOR CONNECTED WITH EMERGENCY SERVICES. IN AN EMERGENCY PLEASE DIAL 911 ON YOUR TELEPHONE.

Kentucky residents: This contract is not an insurance policy and is not protected by the Kentucky Life and Health Guarantee Association.

New York residents acknowledge that the savings offered through the NETWORK is subject to the NETWORK contracts applicable to New York State.

This contract is not protected by the Utah Life and Health Guarantee Association.
